HOSPITAL or REHAB patients must have an approximate date for discharge back home or into a licensed Assisted Living. This is required per Dept. of Elder Affairs Procedures. Rehab, ALF, or SNF placements are not provided by AAAPP. 

Question: What if my patient needs services immediately?

Frequently Asked Question: I am the patient’s Social Worker, why can’t I speak on my patient’s behalf?

Answer: We encourage you to talk with your patient to ask if they would like you to participate on the phone call. With the patient’s permission, we can connect you in via 3 way call. We take person directed care seriously and require that all patients or their legal representative participate in discussions of their own care to the best of their ability. Speak to your specialist if you have a unique situation.
